Admission Steps

Apply to college.
Submit any and all other college and/or military transcripts to Office or Registrar prior to submitting your application.
Submit supporting materials with application packet (forms included in admission packet in above link):
- General application form
- Medical Lab Work Experience documentation (if applicable)
- Previous Program Statement (if applicable)
- Prerequisite Course Information sheet (if applicable)
To submit above application materials:
- Email complete packet to rlarsen@csi.edu or
- Hand deliver to Health Sciences and Human Services Information Office or
- Mail to: HSHS, ��, PO Box 1238, 315 Falls Ave, Twin Falls, ID, 83303
Candidates will be notified by email provided on application of status. Further instructions on next steps will also be provided in an email.
